Discharge from Hospital
Most people who are admitted to hospital are able to go home without needing any further support. However some people need some assistance when they are discharged.
If you have concerns about going home you, or your carers, should discuss them with hospital staff as soon as possible. This will allow time to plan any support services you may need. The hospital social work team will offer you an assessment if you wish. Some people are quite happy to arrange their own long term care.
What if I need help at home?
Home care is a service that you have in your own home. Care workers can help you live an independent life at home. The social work team can organise the care workers to come to help you. This service is not free but you may be entitled to financial assistance.
What if I need a care home?
The outcome of your assessment might be that you need a little bit more time to recover and that going home is not the best place for you once you are fit enough to leave hospital. A short term stay in a care home may help your recovery enough for you to return home after a few weeks. If you and your social worker decide that a residential care home is more appropriate to your needs, you will receive help in finding an appropriate care home.
Remember
Evidence has shown that remaining in hospital beyond the stage of being acutely ill can be damaging to your longer term health. You are at risk of losing your independence the longer you remain in hospital.
If you wish to complain about your treatment in hospital you should raise the matter in the first instance with hospital staff. If you are not satisfied, you can refer your complaint to the Healthcare Commission
